Buy a used Toyota FACTORY service manual, or use the on-line TSRM , and follow the diagnostics.
Toyota manuals have easy to use, step by step instructions on how to get to the bottom of issues with the A340E trans.

The manuals cost about $50 on eBay, and are easier to use than the same thing on the net.
TERM on-line

Most likely a sticking spool in the valve body, or a plugged filter.

When was the last time you had the trans flushed and the filter changed ??
Plugged filters and dirty fluid are the root of most issues on these trans,
The pressure goes down as the filter plugs, which leads to shifting issues, and worn clutch packs.
