#1 - if you are still using the MAF, you will need to run a line to the intake pipe after the MAF.
#2 - IIRC - would have gone to one of the actuators so now that you are single it is unused.
You can either take off the whole metal pipe part and run a line right to the IACV or use the large line like i said above, and possibly use the other for a vacuum source if you need one on that side of the motor
