Did you break it in properly? Clutches should be driven normally for 200 miles before you romp on it. This will allow the material to go through the usual heat cycles and stick nicely to the flywheel. Also, did you check the flywheel runout when you replaced the clutch? Maybe your flywheel is warped from abuse from a previous owner...?

Your slave cylinder double check and make sure it didn't leak on the clutch. Sldo did you have the flywheel turned when it was replaced?
